Long Beach Retina strives to provide easy access and excellent communication to our patients as well as to our referring doctors throughout Long Beach and surrounding communities.
For the convenience of our referring doctors, we provide a referral form (see below) which can be quickly completed and faxed or sent to our office with your patient.
